How to Submit Your Community Service Hours

Keep track of your community service hours using this form. Record the days, times, and hours you volunteer.

At the end of each volunteering experience, do the following:

  1. Have your community service supervisor (at the organization where you volunteer) complete the bottom of this form. Give the completed form to your Guidance Counselor.
  2. Also, submit these service hours online at servo.city (click "Record Your Service," complete this form, and then click "Submit Information"). *HOURS DUE BY MAY 31, 2018 FOR NEW HAVEN PROMISE SENIORS!
